Welcome Email

You will receive a welcome email when you register for a FORMS account. The email will contain
a key and instructions on creating your password.

FORMS passwords require a minimum of 10 characters or more, consisting of at least one capital letter, one lower case letter, one number and one non-letter/digit character, for example, an exclamation mark.

The FORMS Username for either the FORMS portal or the FORMS App, is the email address used to create the FORMS User Account.

When you attempt to log into FORMS, if you enter the wrong credentials 5 times, you will receive an email warning you that 5 failed login attempts have been made. You will be notified by multiples of 5 attempts, for example on 5, 10, 15 20 failed login attempts. Logging in with the correct credentials will permit access to FORMS.

However, if there have been 10 failed login attempts against the same FORMS account, from the
same IP address, within five minutes, FORMS will block the FORMS user account for five
minutes.
